We're happy that your child will be joining us at the Center for Creative Arts! Please fill out a separate registration form for each student. For Registration forms for private music lessons or after-school art classes, please contact our office.  If your child has severe allergy and will be bringing an EpiPen or allergy medicine, we will also need a Severe Allergy Form completed by your physician that details a life threatening allergy action plan. Please contact us if you have any questions regarding which documents your child will need.
